This request form is to be completed by each vocalist when leading worship or performing special music. Form must be emailed to chrisma_luster@yahoo.com by 5pm Monday prior to the Sunday you are scheduled to minister. Wednesday night requests should be sent 1.5 weeks prior to the ministry time.
